ABSTRACT

OBJECTIVE: To describe clinical characteristics, hormonal profile and body composition of obese men in preoperative of bariatric surgery. METHODS: Cross-sectional, population-based study. Patients evaluated from June 2019 to December 2021 in 2 obesity referral centers. Patients underwent clinical evaluation, androgen deficiency screening using Androgen Deficiency in the Aging Male questionnaire, hormonal profile and body composition assessment through body mass index (BMI), body fat percentage (FM-%) and mass (FM-kg) measured by electrical bioimpedance and dual energy x-ray absorptiometry. To characterize hypogonadism, 2 cut-off points were considered: TT <264 ng/dL and TT <164 ng/dL. RESULTS: Thirty patients were included, mean age 35.6 ± 8.8 years, mean weight 129.4 ± 14.0 kg and mean BMI 42.3 ± 4.7 kg/m2. Dyslipidemia was the most prevalent comorbidity. Considering TT <264 ng/dL, 22 patients (73%) had hypogonadism. The mean TT in hypogonadal men was 198.9 + 68.7 ng/dL and in eugonadal men 357.0 + 59.5 ng/dl (P < .001). Using TT <164 ng/dL, 7 patients (23%) had hypogonadism. The mean TT in hypogonadal patients was 116.6 + 28.9 ng/dL and in eugonadal patients 279.0 + 75.0 ng/dL (P < .001). In Androgen Deficiency in the Aging Male questionnaire, 93.3% had positive screening, with no significant difference between groups. There was no statistically significant difference in body composition between groups when using TT <264 ng/dL as the hypogonadism cutoff. Considering hypogonadism TT <164 ng/dL, hypogonadal patients had significantly higher values of weight (139.0 × 126.5 kg P = .036), BMI (46.1 × 41.2 kg/m2P = .014), FM-% (48.0 × 42.8% P = .010) and FM-kg (66.3 × 53.9 kg P = .007) than eugonadal patients. CONCLUSION: Hypogonadism was identified in at least 23% of patients. Considering TT below the lower limit of normality for characterization of hypogonadism, we identified a significant worsening in body composition parameters.

ABSTRACT

O pseudocisto de pâncreas é caracterizado como uma coleção fluida rica em enzimas pancreáticas circundada por tecido de fibrose e granulação, sendo mais comum nos casos de pancreatite crônica e, também, na etiologia alcóolica da doença. Sua indicação cirúrgica se baseia nos casos assintomáticos que não tendem à regressão e nas suas complicações. Entre elas, bastante rara, destaca-se a ruptura espontânea do pseudocisto de maneira livre para cavidade abdominal. Relatamos aqui um caso de ruptura espontânea para cavidade abdominal de pseudocisto pancreático com quadro clínico de abdome agudo com pneumoperitônio no raio-x, que foi tratado com drenagem externa do mesmo para a parede abdominal. Apesar de cada vez mais consagrado o tratamento dessa patologia por via endoscópica ou percutânea, relatamos aqui um raro caso aonde a conduta cirúrgica se tornou imperativa, sendo bastante útil para os cirurgiões.


Pseudocyst of the pancreas is characterized as a fluid collection rich in pancreatic enzymes surrounded by fibrotic tissue and granulation, being more common in cases of chronic pancreatitis. Its surgical indication is based on the asymptomatic cases that do not tend to regression and on its complications. Among them, though very rare, there is spontaneous rupture of the pseudocyst freely into the abdominal cavity. Here we report a case of spontaneous rupture of pancreatic pseudocyst into the abdominal cavity with a clinical picture of acute abdomen with radiologic pneumoperitoneum, which was treated with external drainage to the abdominal wall. Although treatment through endoscopic or percutaneous route is increasingly well established, here we report a rare case in which the surgical procedure became imperative.

OBJETIVO: Relatar a experiência do nosso Serviço com a descompressão cirúrgica da árvore biliar através de uma hepatojejunostomia periférica. MÉTODO: Entre julho de 2000 a julho de 2005, 11 pacientes foram à laparotomia para ressecção de tumores do hilo hepático e, durante o trans-operatório, apresentavam lesões irressecáveis. Os dados analisados foram: idade, sexo, morbidade, mortalidade, dosagem de bilirrubinas séricas pré-operatórias e no 7ºdia de pós-operatório, prurido pré e pós-operatório e sobrevida. RESULTADOS: A idade média dos pacientes foi de 67 anos, seis eram mulheres e cinco eram homens. Icterícia estava presente em 100 por cento dos casos e prurido em 80 por cento. Seis pacientes tiveram o diagnóstico de neoplasia de vesícula biliar e cinco de colangiocarcinoma. Ocorreram três óbitos intra-hospitalares. A dosagem média no pré-operatório de bilirrubina total foi 19,33mg/dl e bilirrubina direta 16,81mg/dl e no pós-operatório 4,88mg/dl e 3,64mg/dl, respectivamente. Oito pacientes que receberam alta hospitalar tiveram sobrevida média de oito meses, evoluindo sem icterícia e prurido. CONCLUSÃO: A hepatojejunostomia periférica pode ser considerada uma boa opção como tratamento paliativo . Ela demonstrou ser segura, com sangramento mínimo, rápida execução e mortalidade aceitável, melhorando significativamente a icterícia, o prurido e fornecendo uma sobrevida satisfatória.


BACKGROUND: This study shows the experience of our service with peripheral hepatojejunostomy. METHODS: Between 2000 and 2005, eleven patients were submitted to peripheral hepatojejunostomy. Laparotomy in all patients had been performed as an attempt for curative resection. We analyzed age, gender, morbimortality, pre and post-operatory dosage of bilirubin, pre and post-operatory pruritus and survival. RESULTS: The mean age of the patients was 67 years, six were female and five were male. Jaundice was present in 100 percent of cases, and pruritus in 80 percent. Gallbladder neoplasm was diagnosed in six patients and cholangiocarcinoma in five. There were three intrahospital deaths. In preoperatory, mean values of total bilirubin was 19.33mg/dl and of direct bilirubin was 16.81mg/dl, and in the post-operatory, 4.88mg/dl and 3.64mg/dl, respectively. The eight discharged patients did not referred pruritus anymore, and their mean survival was eight months. All patients evoluted without jaundice and pruritus. CONCLUSIONS: Peripheral Hepatojejunostomy can be considered as capable to improve quality of life of the icteric patients. Surgery was demonstrated to be safe, with minimal blooding, rapid execution, and acceptable mortality rates. It was possible to improve significatively jaundice, pruritus, and to give a satisfactory survival.
